Activity Sheet Instructions

The Activity Sheet is a summary of activities you have been involved in both in and out of school during grades 9-12. This profile is an important part of your application for college, scholarship and employment consideration as well as assisting you in writing a resume. It is essential that you are thorough and accurate in compiling this Activity Sheet in order to make your first impression a positive one.

The Activity Sheet will be available to download from the computers in the Guidance Office, Career Center, and Media Center. Remember to save all work to a disk, do not save to the hard drive. This will allow you to make corrections and update your activity sheet as needed. If you need assistance please feel free to ask for help in the Career Center, or in the Guidance Office.

Keep in mind:
  • You are responsible for completing or developing an Activity Sheet to be kept on file in the Guidance Office.
    • You are responsible for submitting an updated copy when changes are made.
    • You are responsible for providing this Activity Sheet when requesting a letter of recommendation.
    • You are responsible for submitting your Activity Sheet as soon as possible, and no later than your first transcript request. (By mid-October is best)
  • This is a suggested format. Any neat, organized presentation you use will be accepted.
  • Start with a rough draft, and then organize activities for final copy.
  • The copy you submit will be included in your transcript package as is.
    • If you need help, or suggestions, please see your Guidance Counselor, Mrs. Miller in the Career Center, or Mrs. Stolz, the Guidance Secretary.
  • Cluster like activities together.
  • A notebook listing grades 9 – 12 Academic/Achievement Awards is available for your use in the Career Center.
  • Check at home for your individual in and out of school Awards/Certificates, i.e. Athletic, Art, Music, Theater, etc.
  • Review list with your parents/guardians for their assistance and input.
